Assessment Policies
STUDENT RESPONSIBILITIES
- Review assessment dates at the beginning of the semester in the syllabi, Blackboard shell or other course materials to ensure assessments are scheduled prior to the deadline.
- Schedule assessments during regular course times. Determine with faculty an alternate assessment time if necessary.
- Schedule assessments 96 hours in advance, excluding non-business days (holidays and weekends).
- If the deadline has passed, seek faculty approval for one of the following:
- Schedule assessments with Access Services for the soonest available time.
- Ask faculty to proctor assessments with accommodations.
- Complete assessments in class without accommodations.
- Schedule either time and a half or double time as approved in accommodations.
- Reschedule assessments due to illness/emergency and determine a new date and time with faculty and Access Services.
- When receiving a note card accommodation, complete the following:
- schedule an appointment with faculty to discuss the dimensions of the note card.
- Compose the note card.
- schedule an appointment with faculty, at least 96 hours prior to assessments (excludes non-business days), for content approval.
- Arrive on time for appointments and be prepared to start assessments as scheduled. Arrive early if needing to review material, visit the restroom, fill water bottle, etc. Access Services reserves the right to cancel assessments when students arrive late.
- Check in at the Academic Support Center kiosk and wait for staff outside the Access Services office.
- Bring all required testing aids not provided by the faculty, including scantrons, calculators, pencils, whiteboards, etc.
- Materials supplied by faculty will be distributed at the start of assessments.
- Jackets, backpacks, and other unauthorized materials are not permitted in testing rooms.
- All phones and electronic devices must be stored in provided locked storage during the assessment, unless permitted by accommodations. Ensure cell phones are silent and alarms are off.
- Personal items (i.e. water bottles, fidgets, pencil bags, etc.) may be inspected before permitted in testing rooms.
- Review and sign the Student Assessment Instruction sheet detailing allowable testing aids. Notify proctor immediately regarding questions or concerns.
- A small snack and/or drink is allowed in testing rooms. Full meals are prohibited, unless permitted by accommodations.
- Use only approved testing aids as indicated on the Student Assessment Instruction sheet.
- Tell the proctor before leaving testing rooms for any reason.
- Personal belongings cannot be accessed once the assessment has begun, unless permitted by accommodations.
- Time cannot be paused on assessments for breaks, unless permitted by accommodations.
- A 10 minute warning will be given before time has expired for paper assessments.
- Provide proctor with all materials following assessments.
- Log out of online platform but do not log out of computer.
- Gather all personal belongings following assessments
FACULTY RESPONSIBILITIES
- Monitor Minot State email for scheduled assessments.
- Submit approved testing aids and assessments in Microsoft Word format via email at least 48 hours in advance.
- For standardized assessments, please contact Access Services to coordinate drop-off and pickup.
- Note cards should be sent directly to Access Services when applicable.
- Adjust time settings for online assessments in accordance with accommodation requirements.
- Provide contact information for technical or procedural questions during assessments.
- Respond promptly to communication from Access Services.
- Check Minot State email for completed assessments and any related materials.
- Confirm receipt of assessment materials via email so documents can be shredded. If no confirmation is received, materials will be shredded after three business days.
- Email faculty at least 72 hours prior to scheduled assessments.
- Print assessment materials.
- Set up testing rooms based on necessary accommodations.
- Set up proctoring software when needed.
- Prepare Student Assessment Instruction sheet detailing allowable testing aids for students to review and sign. If questions or concerns arise, contact faculty for clarification. If faculty cannot be reached, the testing aids originally communicated by faculty will be permitted.
- Inspect all personal items (i.e. water bottles, fidgets, pencil bags) before entering testing
- Proctor assessments, using software and surveillance cameras.
- Ensure only approved testing aids are used.
- Contact faculty if issues arise.
- Administer a 10-minute warning before the assessment time expires.
- Track the start and end time of the assessment on the Student Assessment Instruction sheet.
- Report and send evidence of suspected academic dishonesty to the faculty. Students will not be confronted by Access Services.
- Collect all assessment materials and send to faculty via their Minot State email or coordinate drop off and pick up times with faculty for standardized assessments only.
- Shred assessments upon faculty notification of receipt or after three business days.
General Policies
- If students are eligible for flexibility accommodations, they will not be valid until the student sets up a flexibility appointment with Access Services and a Flexibility Accommodation Plan has been sent to the faculty by Access Services.
- Students must renew their accommodations every semester.
- Accommodation letters come from Access Services only.
- If any changes need to be made to a student’s original accommodations during the semester, the student will notify Access Services immediately and meet with the Access Services coordinator to discuss those changes before they are put into place.
- Accommodations need proper documentation for Access Services to substantiate the requested accommodation. Documentation must be from a medical provider, mental health professional, or vocational rehabilitation counselor confirming a diagnosis. Notes written on prescription pads or work, or school excuses may not be adequate. Contact the Access Services Coordinator for any questions regarding documentation at access.services@minotstateu.edu.
- Alternative Textbooks take up to 3 weeks to fill depending on turn around time from publishers.
