Campus Connection Forms
Withdrawal Form
Students who find it necessary to completely withdraw to zero credits must complete the Cancel/Withdraw to Zero form in Campus Connection. More information about withdrawing is available on the Financial Aid Office website.
- Log into Campus Connection.
- Click the eForms GT tile.
- Choose Cancel/Withdraw to Zero on the left.
- Choose Minot State University in the dropdown box.
- Read the information and click the Next button.
- Complete the form and click the Submit button.
Legal Name Change Form
All name change requests must also include the corresponding legal documentation showing the change and a signed social secuirty card reflecting the new name.
Examples of legal documentation include a marriage license or court documents. For international students, the passport name change documentation is acceptable.
- Log into Campus Connection.
- Click the eForms GT tile.
- Choose Legal Name Change Request on the left.
- Choose Minot State University in the dropdown box.
- Complete the form and attach the required documents.
- Click the Submit button.
