Withdrawal Form

Students who find it necessary to completely withdraw to zero credits must complete the Cancel/Withdraw to Zero form in Campus Connection. More information about withdrawing is available on the Financial Aid Office website

  1. Log into Campus Connection.
  2. Click the eForms GT tile.
  3. Choose Cancel/Withdraw to Zero on the left.
  4. Choose Minot State University in the dropdown box.
  5. Read the information and click the Next button.
  6. Complete the form and click the Submit button.

Legal Name Change Form

All name change requests must also include the corresponding legal documentation showing the change and a signed social secuirty card reflecting the new name.

Examples of legal documentation include a marriage license or court documents. For international students, the passport name change documentation is acceptable.

  1. Log into Campus Connection.
  2. Click the eForms GT tile.
  3. Choose Legal Name Change Request on the left.
  4. Choose Minot State University in the dropdown box.
  5. Complete the form and attach the required documents.
  6. Click the Submit button.
