Registrar's Office

Registration Actions and eForm Instructions

Students should be able enroll themselves on Campus Connection once their enrollment time and date has been reached and any time after through the last day to add.

The Registration Action form is an eForm in Campus Connection. Once logged in, click the MiSU eForms tile, then the Registrar's Office folder to access this form.

The automatic notification emails that are sent when you submit or otherwise take action on an eForm will come from donotreply@ndus.edu and you may need to check your junk or spam folders to find them.

eForm Statuses and explanations
Common eForm Statuses What does this status mean? What next?
Executed The eForm has been fully processed and completed. There are no further actions to take.
Pending The eForm is still waiting for approval or being processed.

Depending on the eForm, it may take anywhere from 1 day to 3 weeks to process.

If you have concerns, please contact the appropriate office.
Recycled

The eForm was sent back to the person who submitted it for correction.

There should be feedback explaining the corrections needed in the notification email.
Whoever submitted the eForm needs to make the corrections and resubmit the form, which restarts the approval process.
Denied

The request was denied.

There should be feedback explaining the reason for denial in the notification email.

There are no further actions to take for this eForm.

Depending on the reason for denial, a new or different type of eForm may need to be submitted.
Withdrawn

 The person who submitted the form has canceled the request.

There are no further actions to take.
On Hold

This is different than ‘Pending’ and usually means there is another step that must be completed before the eForm can be processed.

eForms that are ‘On Hold’ typically take longer to update than ‘Pending’ eForms.

Typically, the approver who puts the eForm ‘On Hold’ will provide information on the next steps.

If you have concerns, please contact the appropriate office.

There are three ways to interact with an eForm after it is submitted, Evaluate, Update, or View. Anyone, including students, can update or view an eForm if they were the one who submitted it.

Evaluate an eForm

Only staff and faculty can evaluate eForms.

Recycle You found a mistake in an eForm (wrong date, class number, etc) which needs correction before it can be approved.

Please explain the error and the need to resubmit in the Feedback to Requestor (Recycled) box before clicking Recycle.

The eForm is sent back to the person who submitted the initial form.

They should then edit and resubmit the eForm.

Approve You have verified everything on the form is correct. The eForm moves on to the next approver, or is sent to the Registrar's Office queue to be processed if it has all the required approvals.
Deny You are choosing to deny the request.

Please explain the reason for denial in the Final Feedback to Requestor (Approved/Denied) box before clicking Deny.

The eForm request is terminated, and no further changes or actions can be made.

Update an eForm

Edit and Resubmit You got an email saying your eForm has been Recycled back to you and you need to correct and resubmit it.

Resubmitting an eForm restarts the approval process. The form will go back through each approver needed.

Withdraw (cancel) an eForm

You submitted an eForm in error.

eForms can be canceled as long as they have not been fully processed.

The eForm request is terminated, and no further changes or actions can be made.

View an eForm

View Actions available in eForms
Actions in the eForm When would you use that action? What happens to the eForm next?
View only You want to see the eForms you have submitted (or for faculty and staff, been an approver on).

No actions can be taken in the View tab.

If you are waiting for an eForm to be processed, you can click on Search to find the eForm you submitted, and can see the ‘Form Status.’

  1. The student and advisor discuss the option to enroll in more than 18 credits in the fall/spring or 12 in the summer
    • The student must have a minimum GPA of 3.0 to be eligible
    • Each credit over 18 (fall/spring) and 12 (summer) is charged at a per-credit rate
    • The maximum number of credits students may complete is 22 in the spring/fall and 15 in the summer

  2. The student submits the Overload Request eForm in Campus Connection, entering the course(s) information and their advisor's email as the Approver.
  3. The advisor enters the chair's email and approves or denies after confirming the student's GPA.
  4. If approved by the advisor and chair and if the minimum GPA is met, the Registrar's Office will enroll the student in the class(es).

Course Completion Agreements are due when official grades are due, and will not be accepted after that deadline.

 

  1. The student and faculty discuss the I (incomplete) grade option and assignments left to be completed, agree on the date, and the official grade the student will receive if the work is not completed.

The Graduate School has a more stringent policy and graduate students must complete the work within 60 days of the grade due date. 

  1. The student submits the Course Completion Agreement eForm in Campus Connection, which includes the term, academic career, course information, reason for the request, assignments to be completed, date by which the work must be completed, the grade to change to if work is not completed, and adds the instructor as the Approver. 
  2. The instructor approves, recycles, or denies the student's eForm.
  3. The Registrar's Office team will change the student's grade to I for the course.

  1. The student visits with their advisor about a possible course substitution.
  2. The advisor submits a Course Substitution eForm on the student's behalf, adding the chair as the Approver. 
  3. The chair approves, recycles, or denies the request.
  4. The Registrar's Office team will update the student's academic requirements report with the substitution.
