• Staff and Faculty Flu Vaccine Consent Form can be printed and completed prior to arrival, forms are available at clinics.
• Your form is not complete without your correct Sanford ID number.
• Please DO NOT ATTEND  if you have COVID.

***We will not be able to administer any vaccination without your Sanford Member ID#.

   
Cost
The Student Health Center is only able to bill NDPERS Sanford Health Insurance plans for flu vaccine. There is no out-of-pocket expense for staff that are on the NDPERS Sanford Health Insurance Plan, however, if any portion is not covered, the staff will be responsible for that portion. To be sure that your Sanford Health Insurance Plan covers flu vaccine, please call the number on the back of your insurance card.
