Risk Management Incident Report
Utilize this form to report any injury and/or property damage incident that occurs on campus or at a campus sponsored event.
- The form must be completed by a MSU employee or supervisor
- MSU Security will investigate and report incidents involving visitors to campus
Completed forms should be forwarded to Aundrey L. Livingston Sr. (a.livingston@minotstateu.edu), Safety and Security Director, within 24 hours of the occurrence of the incident.
