CSA Report Form

Campus Security Authorities (CSAs) may use this form to report crimes to Security that fall under requirements of the Jeanne Clery Act. CSAs are required to report to Security any report received involving the below listed offenses if they occur within MSU’s Clery-reportable geography (basically anything that occurs on campus or directly adjacent campus). If there is a question as to whether an incident is reportable under Clery regulations, we urge you to report the incident to Security as soon as possible so that we can decide what the appropriate course of action will be on a case-by-case basis. Timeliness of reporting is crucial, as Security is required to not only post reported crimes on our 60-Day Crime and Fire Log, but we also need to determine if a Timely Warning or Emergency Notification to the campus community is necessary due to the severity of the incident.

  • Criminal Homicide
    • Murder and Non-negligent Homicide
    • Negligent Homicide
  • Sex Offenses
    • Forcible
    • Non-forcible
  • Robbery
  • Aggravated Assault
  • Burglary
  • Motor Vehicle Theft
  • Arson
  • Liquor/Drug/Weapon Law Violations
    • Arrests
    • Non-Arrest Disciplinary Referrals
  • Hate Crimes
    • Includes all of the above listed crimes if there is an element of discrimination involved.
    • Also includes Larceny/Theft, Simple Assault, Intimidation, and Destruction/Damage/Vandalism of Property if there is an element of discrimination involved.
  • Violence Against Women Act Offenses
    • Domestic Violence
    • Dating Violence
    • Stalking
