The following forms are in Word. You must download and save the forms in order to complete them. Please submit forms and any required documents electronically to irbchair@minotstateu.edu
Please submit all documents, including data collection instruments, in a Word version so copies may be retained in the IRB official files. Links to data collection instruments are not acceptable.
