The Approval Process

  1. Researcher Determines Research which Application is Appropriate
    1. IRB Exempt Status Review Form Submitted to IRB Chair
    2. Expedited/Full IRB Application Form Submitted to IRB Chair
  2. IRB Chair reviews forms briefly to confirm that application is complete and correct.
    1. If not complete or correct, email/letter/phone message sent to application for revision.
      1. Application Revised and sent back to IRB Chair for review
  3. If correct, IRB Chair assigns application number and furthers the application
    1. For Exempt applications, Chair confirms Exempt Qualifications
    2. For Expedited applications, Chair submits application to Committee for Review
    3. For Full Review applications, Chair will submit applications to committee for review and inform applicant of IRB meeting the project will be reviewed
  4. Application Exempt, Approved or Conditional Approved
    1. If Conditionally Approved - Any Changes, Corrections, etc. Required by IRB for approval made by Applicant and Submitted to Chair
  5. IRB Approval Letter sent to Applicant
