MSU Veteran's Worksheet - Spring 2015

Complete after you are registered for Spring 2015.

If using Ch. 33 Post 9/11 GI Bill:
1) Have you submitted the 22-1990 Application for Benefits at the VA website ( and if so, when? (If you have already submitted the 22-1990, provide a copy of your Certificate of Eligibility to the Veteran Certifying Official.)

Please contact the certifying official if you last received VA benefits at another school.

Students can only be certified for face-to-face versions of Math 102 (no online versions) and must document a need to take the course by providing either ACT/SAT test scores or a COMPASS test result.

By submission I understand:

  • All course work must be required for my approved degree in order to receive VA benefits; electives may not be eligible for benefits.
  • I can only be paid for repeated courses when the initial grade was an 'F'; VA will not pay for repeats of 'D' grades unless the course requires a 'C' to progress.
  • No payment can be received for audited courses.
  • In case of any failing grade, the instructor will be contacted to verify the last date of attendance.
  • Chapter 30, 1606 and 1607 recipients must verify enrollment monthly (online via WAVE or by phone).
  • I must promptly notify this office of any changes in my enrollment for this semester and that such changes could impact my level educational benefits including potential repayment of benefits (tuition, monthly payment, etc.).
  • I must complete this form each and every semester in which I wish to be certified for VA educational benefits.
  • I consent to the release to the US Department of Veterans Affairs all of my educational records maintained by MSU, including my Social Security Number.
