Criminal Background Check

Prospective Athletic Training Program Students must complete an FBI and North Dakota Bureau of Criminal Investigation (BCI) criminal background check. Clearance must be received prior to admission to the ATEP, thus a copy of your clearance letter must be delivered to the Heather Golly, Program Director of the Athletic Training Program in Swain 108.

Where to get your Criminal History Record Check Request Form:
This form needs to be submitted to the Program Director, Heather Golly, two weeks prior to the October 22 application to the program deadline. The form may be downloaded from this website or may be picked in Swain 218 of the Teacher Education & Human Performance Department. North Dakota Attorney General, and one set of fingerprint cards (two cards).

Where to get your fingerprints made:

  • You may get fingerprinted at the Ward County Sheriff's Office or any other law enforcement center that offers fingerprinting. Payable by cash only (no checks or credit cards.)
  • Ward County Sheriff's Office (Jail Lobby): 315 3rd St. SE, Minot, ND 58701
  • Hours: Tuesdays and Thursdays 7:30-9:30 P.M. There is a $10.00 fee per card, must submit 2 cards ($20).

Who decides at MSU if you can be admitted if you have a criminal record?
The Athletic Training Program Selection Committee will review your materials and render a decision. You may or may not have to appear before the committee.

Renewal per semester:
Each semester students will be required to sign a statement relating to criminal offense form. This form will be provided to you at the beginning of each semester by the ATEP, Program Director.
