Attendance Notification Form
Students are expected to attend all class sessions of any course for which they are registered. Students who are registered for a course but fail to attend any of the first three hours of class meeting time may be canceled from that class by the instructor during the first ten instructional days.
This form will be submitted to the Registrar's Office as consent to drop the student from the course specified. The request must be submitted by the instructor on record.
